I picked up the chalk paint from Wal-Mart. The pots are from Home Depot and I already had the twine and paint brushes on hand.

As per the instructions on the paint container, I did one coat, waited a few hours, did a second coat and then waited 24 hours before curing it with chalk. I purchased the chalk at Sobeys (not my first choice but I happened to be in there already and it was getting down to the wire!). I used my glue gun to secure the twine and then tied the gift tags with the bow. I had scrap booking paper and stickers in my stash.
Here's the final product! Pardon the iPhone photos. I forgot to take "after" shots so thankfully Mom took some and sent them to me.

And there you have it! I love personalized gifts and these were super easy to make.
